Are you doing virtual visits?

No, only in-clinic visits which can be reserved at this page: BOOK APPOINTMENT

Can I get a doctor’s note?

Definitely. Our doctors are happy to write you a doctor’s note for work or school if necessary, and it will be written on their letterhead. There is a $35 fee for doctor’s notes.

Can I get a prescription?

Absolutely! Our doctors are able to prescribe almost any medication, excluding controlled substances.

Can I get lab/bloodwork requisition, or xray/ultrasound requisition?

Yes, we offer lab work requisitions, including blood tests, X-rays, and ultrasound referrals only when deemed necessary by the doctor. The doctor can create a requisition that you’ll be able to take to any lab or medical imaging centre if deemed necessary. *Requisitions are not like ordering something online where you can just demand something yourself; it is up to the doctor if you require/qualify for a test or not, and requisitions for tests can only be provided if deemed medically necessary (this is the law).

Are there any fees?

Patients with a valid Ontario health card will receive OHIP-covered services at no cost. Just like any other clinic, there are fees for uninsured services (services that OHIP doesn’t cover), such as doctor’s notes, forms for insurance company, private physicals, etc. because OHIP doesn’t cover all services doctors can provide.
Patients without OHIP or international travellers/visitors etc. may receive care at our clinic for a fee.

UHIP/Morcare/Private Insurance Patients (i.e. University Health Insurance Plan etc.)

Non-OHIP patients with private insurance plans can be seen and we can fill the claim form. You will need to pay our clinic for the visit fee then we can fill a claim form you provide. We have claim forms for UHIP and Morcare insurance plans ready to fill, but for other insurance plans you need to bring the claim forms.

Patient Support

I tried booking online but it won’t accept my health card number.

Please make sure you enter your valid full health card number and version code into the online booking system. Please note: if you have been to LDRS previously only on Oxford Street and you selected Existing Patient and it’s giving you the error, please book again and select New Patient. This is because the previous location had no electronic system so you would have to select New Patient if asked during online booking. We moved to 595 Wonderland Rd N and we have an electronic medical records system. (If you have had a phone or in person appointment with our new system/location then you would select Existing Patient option). If you are still experiencing issues, please email info@ldrs.ca
